Summary
The current diagnostic pathway for adrenal insufficiency (AI) entails hospital-based tests (early morning cortisol (EMC) or Short Synacthen Test (SST)) which are expensive and resource-intensive, necessitating skilled nurses and a daycare admission. An alternative home-based waking salivary cortisone (WSC) test has been implemented in clinical care in both the adult and children’s hospitals in Sheffield. This has been shown to be an accurate test, is preferred by patients and is cheap and easy to perform. The RE-SCAIT study aims to implement the WSC test in 4 geographically disparate NHS Trusts and to evaluate the success of this implementation.
What we plan to do
Project Aim:
To evaluate the implementation and uptake of WSC for adrenal insufficiency testing in adults and children referred for diagnosis of adrenal insufficiency in the NHS.
Objectives:
- To develop an implementation strategy for WSC for adults and children.
- To identify adaptations needed to improve inclusiveness.
- To measure clinician uptake of WSC as a replacement for conventional testing.
- To identify facilitators and barriers to implementation in different types of hospital.
- To assess the costs of offering WSC versus current testing for adrenal insufficiency.
- To refine the implementation strategy to facilitate widespread NHS rollout of WSC, if appropriate.
